To seperate the conversations between things learned and sign-ups, this is my take on the previous run (and I'm sure will echo among other comments). This isn't based on one thing and I think in general everything will go smoother next time now that some people have a better sense of exactly how the fights are (me included).
No talking in vent channel outside of designated raid leader and assist - with focus on what's happening in the raid.
Raid leader (whoever it is) needs a mic (I need to go buy a usb one, lol)
Everyone needs Omen or KLH Threatmeter - this is a requirement so that everyone can track threat both from healing, dps, etc.
Everyone stands back with only puller at the front, no mistaken body pulls
During pulls mobs get sorted out by tanks, etc. (split apart) and then dps starts in - too many people in one area at the same time results in mobs being missed, etc
Review of the raid marks - everyone needs to be clear on order of mobs
Enforce some toon mix - I would rather run a heroic then not have the right mix. Personally for the first part I'd say that two undead CC's are required (hunter/priest) - and maybe put Lem near the healers so that if someone breaks, he can frost nova to slow them down
Practice using focus - basically lets you track something permanently so you might set the focus to be the tank and then assist the tank on whoever his target is - that, used with target of target (which works on your primary target and your focus) works great.

It can be very useful for healers or mages. Some addons provide untiframe specific for focus target.
